Our ambulatory surgery unit had been experiencing an unacceptable amount of delays and cancellations due to incomplete charts. Missing or unfinished lab work, pre-op testing and history and physical documentation resulted in gaps in the OR schedule, lost revenue and frustrated patients and families.
