Can You Spot These 10 Drug Errors?

By closely examining these photos, you'll learn how to prevent medication hazards in your facility.

Mislabeled vials, sloppy drug orders, disorganized supply shelves: It's the little things that can lead to big medication errors in the perioperative setting. I recently spent several months visiting ambulatory surgery facilities and snapping photos of poor medication safety practices that, in many cases, were disasters waiting to happen. Examine each of the following photos and see if you can identify what's wrong before reading on to find out how to avoid similar mistakes and improve medication safety at your facility.
