In a matter of months, the somewhat routine practice of endoscope reprocessing became anything but. Last fall, the FDA, CDC and Department of Veterans Affairs issued a joint advisory cautioning healthcare facilities about the potential risks to patients if flexible endoscopes and their accessories were processed improperly. Then, in early December, the FDA issued a safety alert that advised users of the Steris System 1 liquid chemical processor to find alternative means of instrument sterilization and disinfection.
