Ambulatory Surgery Fall Risk Assessment Tool

Our hospital based Ambulatory Surgery Dept is working to develop a Fall Risk Assessment Tool. Is there anyone out there that has a tool that they use that works well for them that they would be willing to share with us. Also, do you assess every patient?

We assess every patient in our preop questionaire for fall risk and ask them if they have a history of "dizziness, falling, or vertigo." Our patient population is elderly, and we get quite a few positive responses to this question. We then hilight their name band, and that notifies other staff members of the patients fall risk history, and to assess for wheelchair use while in our ASC. J. Bell, RN

As part of AmkaiCharts, our EMR designed for ASC's, we use the Morse Fall Risk Scale. You can download PDF's of the information. AmkaiCharts is designed to have the assessment completed by department as the patient progresses through the procedure to allow for changes in orientation.
