Patient discharge after MAC for cataract patients
What is your policy for patient discharge after cataract surgery when the patient receives sedation for the procedure? Must the patient have responsible adult take them home? What if the patient wants to take a cab or bus home?

We are also AAAHC accredited and follow the CMS Conditions of Coverage. The physician offices provide written information prior to arrival and we have the patient sign a post discharge instruction sheet that acknowledges at discharge the patient will be discharged to the company of an adult, who will be responsible for them upon discharge and preferably remain with them for 24 hours or at least until the sedation has worn off. If the patient takes a taxi or bus, they still need an adult companion to accompany them home with whatever means of transportation they chose. Cataracts are done in our facility primarily under topical (eye gtt) anesthesia with a minimal amount of IV Midazolam administered by a CRNA, typically 1 mg, prior to transfer to the OR. Patients commonly return to PACU fully awake postop and are monitored for brief period, approximately 15-30 minutes prior to discharge. We do, however, require they are accompanied by a responsible adult who acts as driver and also understands the patient's discharge instructions. We follow CMS's Condition of Coverage Q-0267, 416.52(c) "The ASC must ensure all patients are discharged in the company of a responsible adult, except those patients exempted by the attending physician." We are AAAHC accreditied and they also require a responsible adult - see Patient Rights - G3 and Aneshtesia - O. Our policy encourages the responsible adult to remain with the patient until "the effects of the sedation have worn off." Many of our physicians in GI tell their patients in pre-op that they may do the procedure without sedation if the patient insists on driving themselves home. Our patients are required to remain in post-op for 15 minutes, v/s must be stable or addressed according to esstablished criteria. They must have a discharge order by an MD. We follow CMS's Condition of Coverage Q-0267, 416.52(c) "The ASC Must - ensure all patients are discharged in the company of a responsible adult, expect those patients exempted by the attending physician." We note the name and relationship of the person accompanying the patient on the discharge sheet. Our ASC policy also addresses this criteria for dismissal. All of our patients who receive sedation are required to have a responsible adult take them home. If we know there is an issue before surgery we will try to arrange for some other service to deliver them home, if they show up and do not have a ride we will cancel their case. C. All patient's receiving sedation require a responsible adult present to sign discharge papers and accompany the patient home. Our patients may ride in a health care provided taxi alone, but we encourage them to have some one home with them, or someone they can call if they need something. Our patients receive sedation and are required to have a responsible adult present to take them home. They may take public transportation or a taxi if they have a responsible adult with them. Our cataract patients all receive sedation and are requied to have a responsible adult to accompany them home. we have had patients use cabs and bus transportation, however, we require a responsible adult to accompany them on the bus.
